I verify the information I have given for myself and child(ren) was truthful. I understand falsifying any information on this registration will result in my child being removed from this program with absolutely no refunds being given. Teams are formed by SCHOOL and by AGE/GRADE APPROPRIATENESS. The league does reserve the right to request proof of school, grade, or age.

Minor Waiver/Release for Buffalo Athletics Inc. activities. RELEASE OF LIABILITY FOR MINOR PARTICIPANTS READ BEFORE ACCEPTING IN CONSIDERATION OF my child/ward, being allowed to participate in any way in the Buffalo Athletics Inc. related events and activities, the undersigned acknowledges, appreciates, and agrees that: The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 1) FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child’s participation; and, 2) I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child’s readiness for participation and/or in the program itself, I will remove my child from the participation and bring such attention of the nearest official immediately; and, 3) I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Buffalo Athletics Inc. its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers and if applicable, owners and lessors of premises used to conduct the event (‘Releasees’), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child’s involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 4) I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. By checking the box labeled ‘I accept the waiver’ and INDICATING YOUR ACCEPTANCE OF THIS AGREEEMENT AND WAIVER, YOU ARE AFFIRMING THAT YOU HAVE READ AND UNDERSTAND THIS AGREEMENT AND WAIVER AND FULLY UNDERSTAND ITS TERMS. YOU UNDERSTAND THAT YOU ARE GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE. YOU ACKNOWLEDGE THAT YOU ARE SIGNING THE AGREEMENT AND WAIVER FREELY AND VOLUNTARILY, AND INTEND BY YOUR ACCEPTANCE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.

I give Buffalo Athletics Inc. permission to disclose in Buffalo Athletics Inc. related newsletters, brochures, websites or other media and photographs, video, audio or other materials which I or my child(ren) may have appeared, spoken, written or otherwise represented. Acknowledging this releases representatives of Buffalo Athletics Inc. to use any of this material.

The goal of the Buffalo Athletics Inc. is to provide a safe, fun learning environment for the youth of our community. We strive to create an opportunity for kids to work together as a team toward common goals. We emphasize sportsmanship, teamwork and respect for authority figures (Board Members, Coaches, and Game Officials). Our focus is on developing good life skills through the sports of football and cheerleading. Buffalo Athletics Inc. will not tolerate verbal or physical abuse of its volunteer coaches, referees, or players from any Parent, Player, or Spectator. Spectators, as well as the players and coaches, are expected to abide by the code of conduct described here at all Buffalo Athletics Inc. events. While most of the adults and children in the program will abide by this code without being instructed, it is being published to protect the children, the integrity of our program, and emphasize the values of Buffalo Athletics Inc. Athlete’s Code of Conduct I Will: – Have fun! – Be a good sport (win or lose), be honest and fair. – Learn the value of commitment to the team; sportsmanship, ethical conduct, and fair play. – Show courtesy and respect to my teammates, opponents, coaches and officials. – Know that athletic contests are educational experiences. – Give complete attention to the instructions of my coaches and league officials. – Maintain self-control during all practices and games. I Will Not: – Use profanity, talk trash, or taunt others before, during, or after any game. – Display any unsportsmanlike behavior. – Criticize my teammates or players from other teams. – Act in any way that may incite spectators. – Engage in horse playing of any kind during any practices and games. – Engage in bullying of any type. Parents/Spectators Code of Conduct As a parent or spectator, I will: – Support the coaches, players, and/or cheerleaders and help teach the value of commitment to the team, sportsmanship, ethical conduct, and fair play. – Support my child’s team and help my child and Buffalo Athletics Inc. make athletic contests a positive educational experiences – Direct constructive criticism of my child’s team program to the head coach and/or association officials at an appropriate time. I will work toward a positive result for all concerned. – Accept decisions of the game officials (including coaches) on the field as being fair and called to the best ability of said officials. The officials are in charge of all games; their decisions are final. – Exhibit exemplary sportsmanship at all times. – Emphasize that good athletes strive to be good students that are physically and mentally alert. – Strive to make every football/cheerleading activity serve as a training ground for life, and a basis for good mental and physical health by being present to support the children. – Abstain from being under the influence of or in possession of and/or drinking alcoholic beverages and the possession or use of any illegal substance on both the game and practice fields. – Stay behind the designated spectator line during a game and practices. – Direct my concerns/complain privately, abiding by the 24 Hour Rule and Problem Resolution/Grievance Policy either by email or verbal to the Head Coach. I will not: – Interfere/interrupt the coaching staff during games or practices. – Criticize officials or coaches, direct abuse or profane language toward them, or otherwise subvert their authority. – Criticize an opposing team, its players, coaches, or spectators by word of mouth, by gesture, or by deliberately inciting unsportsmanlike behavior. – Undermine, in work or deed, the authority of the coach or Buffalo Athletics Inc. program. – Use at any Buffalo Athletics Inc. events tobacco (including vapor or e-cigarettes), drugs or alcohol. – Be under the influence of alcohol or any drug related products. Zero Tolerance Policy At any Buffalo Athletics Inc. event, practice, game, or competition, any adult who verbally abuses, attempts to intimidate, is flagrantly rude to, or does not control their language or actions with a player, official, coach, spectator, or Buffalo Athletics Inc. Board member will be asked to leave the event. He or she will receive written warning regarding their behavior. If the spectator fails to leave upon request, law enforcement authorities may be called to remove the spectator. Any adult that commits a second similar offense will be banned from all Buffalo Athletics Inc. events (home and away) for one calendar year. Any adult who physically assaults an official, coach, Buffalo Athletics Inc. Board Member or another adult at a game, practice or event will be banned from Buffalo Athletics Inc. for one year from the date of the offense. After one year, the adult may apply for re-instatement and will required the approval of the Board. If the adult commits a second offense, he/she will be permanently banned from all Buffalo Athletics Inc. events and property including away games/events. The term physical assault includes, but is not limited to: hitting, slapping, pushing, spitting, kicking or striking in any way with any part of the body or any physical implement. Problem Resolution/Grievance Policy When a parent has a complaint, the following procedure should be followed to attain resolution: 24-Hour Rule Before contacting your Head Coach, Assistant Coach, Commissioner of Football, Buffalo Athletics Inc. President or any Buffalo Athletics Inc. Board member with complaints regarding on-field activities, playing time concerns, etc. you are required to observe the 24-Hour rule and wait a complete day before reaching out to any of the above mentioned people. Additionally, Buffalo Athletics Inc. will NOT address any third party or anonymous phone calls, emails, social media messages, etc. Step 1 Communicate the problem to the head coach and attempt to come to a resolution. Pick a time that is appropriate to talk calmly, privately, and candidly with the head coach. The head coach or parent may, however, ask one other parent or coach to join the conversation as a witness. Step 2 If the problem cannot be resolved, the parent and/or coach need to communicate the issue to the Buffalo Athletics Inc. Commissioner/Assistant Commissioner Football. Step 3 In the event the problem has still not been resolved, the parent can contact the Buffalo Athletics Inc. President. The problem must be submitted in writing with the results from steps 1 and 2 documented. Buffalo Athletics Inc. President will NOT address any issue until steps 1 and 2 have been completed and the results documented in writing. Buffalo Athletics Inc. reserves the right to refuse registration to any child based upon a history of non-conformity to established Buffalo Athletics Inc. rules. The Buffalo Athletics Inc. Board will make every attempt to resolve all matters, as they deem fit or necessary. The Buffalo Athletics Inc. Board’s decision is final on all matters relating to Buffalo Athletics Inc.

Bullying of any kind is unacceptable at Buffalo Athletics Inc. and will not be tolerated. Bullying is counterproductive to team spirit and can be devastating to a victim. The Program is committed to providing a safe, caring and friendly environment for all of our athletes. If bullying does occur, all athletes and parents should know that incidents will be dealt with promptly and effectively on a case by case basis. Anyone who knows that bullying is happening is expected to tell a coach, program owner or athlete/mentor. Objectives of the Program’s Bullying Policy and Action Plan: 1. To make it clear that the Program will not tolerate bullying in any form. 2. To define bullying and give all program coaches, parents and athletes a good understanding of what bullying is. 3. To make it known to all parents, athletes and coaching staff that there is a policy and protocol should any bullying issues arise. 4. To make the process of how to report bullying clear and understandable. 5. To spread the word that Buffalo Athletics Inc. takes bullying seriously and that all athletes and parents can be assured that they will be supported when bullying is reported.

IMPORTANT! Attendance and Commitment is very important for our flag program to run successfully. Buffalo Athletics Inc. keeps team sizes small in order to allow everyone to have ample playing time and get involved in the games. When a coach is constantly missing multiple athletes it makes it impossible to run a practice and play in a game. We understand that some athletes are multi-sport athletes and may have to miss practice or a game occasionally. However, We ask that you do not take a spot on a team if you cant commitment to atleast 85% of the season. Your coach has the right to replace an athlete or limit playing time if your athlete is a no show to any event.
